You see this all over Guatemala. It seems to be a requirement of womanhood to be able to carry a basket on your head without holding on to it. They do take a piece of thick cloth and roll a circle of cloth to serve as a base. Young girls, learning the skill, will often steady the basket with a single hand. Professionals never even think about it. They'll step up and down off of curbs, go around barriers and cross cobblestone streets without batting an eye. We've even seen woman tying their aprons while walking down the street with baskets on their heads. Sometimes you'll see other items carried on their heads. Once we saw a large igloo ice chest being carried on a woman's head down a dirt path along the Central American Highway. Now that was a real Guatemalan woman!
